Registered Nurse - CBOC PACT

This individual provides care and service to adult patients, aged 18-85 and older, who have chronic health conditions including hypertension, diabetes, respiratory disease, arthritis, cardiovascular disease, cancer and substance abuse; in addition to face to face visits with VA providers, this nurse will also provide care utilizing Clinical Video Telehealth equipment to facilitate primary care remote visits. Duties of this position include but are not limited to:
Providing a full range of nursing care to patients, assuming responsibility and accountability for nursing actions. Completing initial assessments and reassessments of patient condition and response to treatment and other clinical findings. Initiates follow-up as needed. Assessing patient's learning needs, learning preference and readiness to learn, develops and implements a plan to meet needs, and instructs patients and families. Collecting laboratory specimens ordered by Licensed Independent Practitioners (LIP). Assessing patient and family learning needs, instructing patient and family on self-care, disease prevention, health promotion, exit interview activities taking into account issues related to patient's age and cultural diversity concerns related to religion, ethnic background or other issues. Providing therapies and treatments based on Nursing assessment, LIP orders, protocols, Basic cardiac life support protocols, standards of care and in compliance with facility references. Administering medication in accordance with established policy/procedure. Administering the patient site facilitation of the clinical and technical aspects for real-time Telehealth applications for Teleprimary Care clinics to connect patients and primary care providers for delivery of primary care services. Displays a helpful, proactive attitude towards internal and external customers in answering questions and resolving problems to enhance patient and customer satisfaction within applicable VA Policy and guidelines. Delegates appropriate clinic activities to licensed practical nurses, medical technicians, Student Nurse Technicians in accordance with competencies of the delegate and limits of licensure and VHA Regulations. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant. NOTE:
See exception in subparagraph (b) below for candidates who are enrolled in a MSN Bridge Program. The Accreditation Commission for Education in Nursing (ACEN), previously known as the National League for Nursing Accrediting Commission (NLNAC) OR The Commission on Collegiate Nursing Education (CCNE). The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. In these programs, students are given a certificate of professional nursing to sit for the NCLEX and should submit this certification to VA prior to appointment. Students do not earn a BSN, but receive a MSN degree upon completion of course work. A copy of the MSN transcript must be provided to VA. Bridge programs that confer a master's degree also fully meet the education requirement, even though a Bachelor's Degree is not awarded. OR In cases of graduates of foreign schools of professional nursing, possession of current, full, active, and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. Registration:
A registered nurse (RN) will have a current, full, active and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the U.S. or in the District of Columbia. The appointing official may waive this registration if the RN is to serve in a country other than the U.S. and the RN has registration in that country (e.g., Philippines). The RN must maintain a current, full, active and unrestricted registration to continue employment with VA. English Language Proficiency:
RNs appointed to direct patient care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d). Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR An ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR A Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR An ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR A BSN with approximately 1-2 years of nursing practice/experience; OR A Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR A Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR A Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR A Doctoral degree in nursing or related field and approximately 2-3 year's of nursing practice/experience. Physical demands include repetitive body movements including the following:
frequently reaching, standing, walking, and lifting of equipment and/or patients to provide patient care and assessment. This position requires extension of the hands and arms in any direction including above or below shoulder level, during the provision of care and assessment, including retrieval of needed equipment, supplies and to make contact with the resident. This position requires the maintenance of an upright position to perform duties associated with patient care. This positions is typically non-sedentary, and frequently involves being in the standing position. This position requires moving about on feet to accomplish tasks. This position requires the raising of objects from a lower position to a higher position or the movement of objects horizontally position to position. Lifting may be done to move patients or supplies from one location to the next. This position requires exerting up to 35 pounds of force occasionally and/or up to 15 pounds of force frequently to move objects. This position requires writing or otherwise working with fingers and/or hands, including one or both hands frequently including keyboard use to enter information into the computer, etc. This position requires substantial movements (motion) of the wrists, hands and/or fingers, as is done when entering patient notes into the computer.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
